Massachusetts Route 181 begins at an intersection with US-20 in Palmer and crosses the downtown section of Palmer. It crosses under the Massachusetts Turnpike without an interchange, the Ware River, Three Rivers, and Bondsville. At the north of Bondsville, in turns and leaves Palmer and Hampden County, and continues into Belchertown in Hampshire County. It crosses rural and residential Belchertown and eventually to the downtown area. In this downtown, it intersects with US 202/MA 21 in their consignment. It intersects with no other routes during the journey, as it only crosses the Mass. Pike.
